Job Posting: Hospice SOC PRN RN

Visiting Nurses Association (VNA) Position Type: PRN (As Needed) Schedule: Scheduled days per week - TBD Minimum Availability Requirement: Must be able to work a minimum of 10 hours every 90 days

Position Summary

Visiting Nurses Association (VNA) is seeking an experienced and compassionate PRN Registered Nurse (RN) - Hospice Start of Care (SOC) to provide skilled nursing services to patients in their homes. This role is responsible for completing comprehensive Start of Care, visits, while supporting high-quality, patient-centered care in accordance with physician orders and regulatory requirements.

Key Responsibilities

  • Conduct comprehensive nursing evaluations and routine skilled visits as assigned
  • Develop, implement, and update individualized plans of care
  • Perform skilled nursing interventions, assessments, and patient education
  • Collaborate with physicians, therapists, social workers, CNAs, and the interdisciplinary care team
  • Ensure compliance with Medicare, state, and agency regulations
  • Complete documentation accurately and within required timeframes
  • Participate in required compliance training and departmental development activities

PRN Pay Guidelines

  • Admission (SOC): $150.00 per visit
  • RN Visit: $75.00 per visit

Qualifications

  • Current, unrestricted Registered Nurse (RN) license in the State of Kansas
  • Minimum of one (1) year of RN experience required; hospice experience strongly preferred
  • Current CPR certification
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong assessment, documentation, and time-management skills
  • Ability to work independently while collaborating with a multidisciplinary team
  • Commitment to providing compassionate, high-quality home health care
